Members forms are available from Service Points
(except Derrys). Alternatively, we have reproduced them on this web site so you can
print them out and complete them at home. When completed, please hand in at a Service Point who can also help you to fill them in, or post to us.
Application to Join the Credit Union
An adult application form is available by clicking here.
Note that it is a requirement of the Financial Services Authority (FSA) that we positively
establish the identity of all members, so you will need to bring along the completed form
and proof of identify personally to one of our Service Points.

Standing Order Form
Using this form, you can arrange for your bank
to make regular (weekly or monthly) payments to your share and loan accounts with the
credit union. Please send to us rather than direct to your bank so we can
ensure the right payment reference is included with your regular payment. (Note there are
two copies of the form on each A4 sheet - you only need one!)

Share Withdrawal Form
The Share Withdrawal is available by clicking here.
If you have a loan, there may be restrictions on the amount you can withdraw from your
share account - for details, please contact us.
Junior Saver Withdrawal Form
Junior savers have a special withdrawal form available by clicking here. Junior savers below 7 years of age do
not need to sign this form (but they can if they want). Parents or guardians MUST sign all
withdrawals.
Deposit Slip
Blank deposit slips are available at Service Points for you to complete them when
making payments into your passbook. Alternatively, you can print them by clicking here and cutting into 3. Don't forget to bring
your passbook with you to the Service Point as well.
Loan Renegotiation Form
If you have a loan, you can use this form to
change the terms agreed when you took it out. There are restrictions to the changes you
can make, so we strongly advise you to contact
us before completing this form.
Share to Loan Transfer Form
If you have a loan, you can reduce it by using this
form to transfer some of your shares to your loan. There are restrictions to the
amount you can transfer, so we strongly advise you to contact us before completing this form.
Member to Member Transfer Form
You can use this form to transfer shares
from one member to another. If you have a loan, there may be restrictions to the amount
you can transfer in this way, so we strongly advise you to contact us in this case.
